The Grey’s Anatomy universe is expanding — this time, with a series set down south. ABC announced on Tuesday that it had greenlighted a brand-new spinoff series of the beloved medical drama from Shonda Rhimes, the original show’s creator and executive producer, and Meg Marinis, who has been the Grey’s showrunner since Season 20. Rhimes and Marinis will be credited as co-creators of the spinoff.
The untitled medical drama has received a straight-to-series order and is slated to debut in midseason 2027, according to Deadline. Grey’s star and executive producer Ellen Pompeo will also serve as an executive producer for the new offshoot.
The official Grey’s Anatomy Instagram account confirmed the news in a joint post with ABC, alongside the caption, “Just what the doctor ordered. 🩺🤠.”

The spinoff, which comes more than 20 years after the original series premiered, will be produced by 20th Television and Shondaland. It will mark the fourth installment of the Grey’s Anatomy franchise, following Private Practice and Station 19, and the first that is set neither on the West Coast nor in a big city. Grey’s Anatomy and Station 19, as fans know, are set in Seattle, while Private Practice took place in Los Angeles.
Per Variety, the hourlong series is “an edgy drama about a team at a West Texas rural medical center — the last chance for care before miles of nowhere.” While the show will revolve around a brand-new crop of doctors, Deadline reported that it’ll likely still be part of the wider Grey’s Anatomy universe.
“I am incredibly excited to expand the Grey’s Anatomy universe,” Marinis said in a statement to Deadline. “This opportunity will bring new characters and stories to life that will embody the same heart, emotion and connection audiences have loved from Grey’s for more than two decades — all set in my home state of Texas. I am so grateful to Shonda Rhimes for creating this dynamic world and feel so fortunate that I get to be a part of it.”
The offshoot’s season premiere will mark the first episode that Rhimes has written or cowritten since the Season 11 episode “How to Save a Life,” in which Derek Shepherd (Patrick Dempsey) died, according to Deadline.
Grey’s Anatomy, ABC’s longest-running primetime scripted series, recently concluded its 22nd season. The show is set to return for Season 23 in the fall.
