Former child actor Blake Garrett’s cause of death has been revealed, five months after his death was first reported in February
Blake was known for his role in the 2006 children’s comedy How to Eat Fried Worms, but he did not continue acting after the movie’s release

Former child actor Blake Garrett’s cause of death has been determined, more than five months after he died at 33.
Garrett was known for his role in the 2006 family comedy How to Eat Fried Worms. TMZ first reported his death on Monday, Feb. 9. His mother Carol Garrett told the outlet he died Feb. 8, in Tulsa, Okla.
On Monday, July 13, the Tulsa chief medical examiner’s office announced that Garrett’s probable cause of death was acute fentanyl toxicity. There were no other significant conditions listed that may have contributed to his death, and Garrett’s manner of death was declared an accident. According to the chief medical examiner’s report, Garrett was found at his residence in Tulsa on Feb. 8 around 9:44 p.m. and pronounced dead at the scene.
According to TMZ’s reporting in February, Carol said Blake had recently visited an emergency room after experiencing intense pain and was diagnosed with shingles in the weeks before his death. She said that he had embraced a sober lifestyle in recent years. The chief medical examiner’s report noted that Blake was living at a sober living house at the time of his death.

Blake is credited with two onscreen roles as a child actor, in the 2004 Barney & Friends special Barney’s Colorful World, Live! and as the character Plug in 2006’s How to Eat Fried Worms, based on the 1974 children’s book by Thomas Rockwell. How to Eat Fried Worms follows Billy (Luke Benward), a boy who finds himself making a bet with classroom bully Joe (Adam Hicks) that he must eat 10 worms without vomiting after starting at a new school.
“I play the bully’s henchman,” Blake told Oklahoma City-based newspaper The Oklahoman in a 2006 interview when he was 13. “But everybody got along on the set.”

The other How to Eat Fried Worms child actors included Hallie Kate Eisenberg, Alexander Gould, Ryan Malgarini, Ty Panitz, Philip Bolden, Andrew Gillingham, Austin Rogers, Nick Krause, Stephan Bender and Alexander Agate.
The movie’s adult cast included Tom Cavanagh, Kimberly Williams-Paisley, James Rebhorn and Andrea Martin.
Blake did not continue with screen acting after How to Eat Fried Worms. When he spoke with The Oklahoman about his experience in the movie back in 2006, the newspaper reported that he was interested in playing football and wrestling.
